Welcome

Hive Athlete
Intake Form

You're here because you're serious about your training — and so are we. At The Hive, we integrate mindset, nutrition, training, and recovery so you can find consistency, experience joy in sport, and ultimately perform at your best every day.

We intentionally consider female physiology in our programming where it's relevant to you. Everything you share is confidential within the coaching team and used only to build the best possible program for you.

In the next sections, we'll explore your life stress, nutrition, training history, current baseline, and goals so we can design a program that actually fits your real life.

This helps us tailor any events or offerings directly to you.
Include country code if outside Canada.
Before We Begin

Not Your Typical
Intake Form

Nothing at The Hive is typical, and this intake form isn't either. Integration of mindset, nutrition, training, and recovery takes intention — and as a Hive member, it starts now.

YOUR COACHES ARE COMMITTED TO:

  • Developing body awareness, building body literacy, and helping you take action so you can navigate the challenges of sport and life.
  • Consistent training that supports your performance potential and your long-term health.
  • Joy in training and in racing (if you choose to race).

We take this so seriously that we actually study it. With your permission, we may use your responses, your training data, and de-identified notes from meetings to evaluate what's working and improve our systems.

Your information is stored securely, and you will always remain anonymous in any aggregated or research reporting.

Do you consent to The Hive using your intake responses, training data, and de-identified coaching notes in aggregated and anonymous form to evaluate and improve our programs?

A Note From Us

Thank you for
sharing this honestly.

One of the things we know about metabolic health — and when appropriate, female physiology — at this stage of life is that more load is not always the right answer. Training harder through a depleted state makes things worse, not better.

We will review your full intake and be in touch about your starting point. We have a specific program designed to rebuild your metabolic foundation before layering in sport-specific load — and it's real training, not a rest protocol.

Continue to share your training preferences so we can build the complete picture.

Pelvic Floor Health

You do not have to submit this portion of the questionnaire — it is however important information to have.

Pelvic floor dysfunction is rarely discussed and often ignored. Yet in one study, 50% of women experienced stress urinary incontinence. While it is outside the scope of practice of The Hive to make specific recommendations, we endeavour to educate all female athletes on pelvic floor function.

This screening tool comes directly from the Pelvic Floor First Screening Tool created by the Continence Foundation of Australia. If you experience any symptoms outlined, speak with your doctor or a pelvic floor physiotherapist — many of these symptoms can be treated and in many cases cured.

This section is optional. Continue when ready.

We'd love to feature you on the Hive athlete page.

e.g., "I train full-time around family and work, and I'm #InMyElement when I'm climbing a long gravel road with nothing but the wind and my legs."
